Updated Policies / Client Waiver

  • Due to COVID-19, we are temporarily limiting the number of daily appointments. The health and safety of our clients and staff is very important to us. For this reason, walk-in appointments will not be accepted and clients who are not currently receiving a service will be asked to step out in order to control the number of people within the salon/spa/clinic. If you are experiencing a fever, cough, or sore throat, please reschedule your appointment for when you are no longer symptomatic. If you have been to a COVID-19-impacted area or have been in close contact with a person infected with COVID-19, we ask that you please reschedule your appointment for 14 days past the date of contact. Please note, we are requesting that clients wear face coverings when they arrive for their appointments.  Your intake will be sent to you by email and must be completed online 24 hours before your appointment.

  • In accordance with extended shelter-in-place recommendations to protect our more vulnerable populations, I am not working with clients with compromised immune systems, clients aged 65 or above, or clients in other elevated at-risk categories at this time.

  • If you are pregnant you certify that you are not considered high risk and your doctor has given the clear for you to receive massage therapy.  You understand that if you are high risk, you are required to bring a doctors note.

  • Cancellation Policy:  Amid the ongoing uncertainty of COVID-19, we have modified our cancellation policy to offer greater flexibility to all our clients. We hope this will alleviate any stress and hesitation you have about an upcoming appointment.  If you need to reschedule for whatever reason, and especially if you are not feeling well, we understand and request for you to please contact us as soon as possible to reschedule.  To further support you, there will be no penalties for cancellations.

  • No Call/No Show:  if you choose to forgo your appointment for any reason and do not call to cancel or reschedule, you will be charged for the full amount of the appointment and may be required to prepay for future appointments.  We require 24 hours notice to cancel or reschedule an appointment.

  • Late Arrival:  If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, your therapist will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for the full session. Out of respect and consideration to your therapist and other customers, please plan accordingly and be on time.

  • Sickness: Massage is not recommended when you are sick. For both the safety of the massage therapist, the client, and other clients, please contact your therapist as soon as possible to reschedule/cancel your appointment.  If you arrive to your appointment sick, you will be rescheduled and charged a rescheduling fee of 50% of your session cost.

  • Weather Policy: in the case of severe bad weather, the massage therapist or client may cancel the appointment.  Please try to give at least 2-4 hours notice.

  • Gift Cards/Certificates:  Gift Cards are not redeemable for cash/credit/products. Services that exceed the value of the certificate will require additional payment. No shows/cancellations with less than 24 hours notice will be charged for the full scheduled appointment. Idaho Code §14-514

  • Groupon:  Your Groupon voucher number is required to hold your appointment.  Due to COVID-19 all Groupon Vouchers will be redeemed automatically 24 hours before your appointment.

  • Full draping will be used at all times during the massage.  Only the area being worked on will be undraped, according to state licensure laws.

  • The massage you receive is provided for the purpose of relaxation and/or relief of muscular tension. Massage should not be construed as a substitute for medical examination, diagnosis, or treatment. Your massage therapist is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat any physical or mental illness. Any illicit or suggestive remarks or advances made by you will result in immediate termination of the session, and payment will be due in full. The License Massage Therapist reserves the right to refuse or terminate massage session to anyone whom she considers to have a condition for which massage is contraindicated.

  •  I understand that, because massage therapy work involves maintained touch and close physical proximity over an extended period of time, there may be an elevated risk of disease transmission, including COVID-19. By signing this form, I acknowledge that I am aware of the risks involved and give consent to receive massage and bodywork from this practitioner.

 

Considerations for Outcall Appointments

 

  • Travel Fee -  A minimum $25 charge will occur if treatment location is over 5 miles from Boise ID 83703.

  • Parking - Client is responsible for any parking fees that occur for their appointment.  Please inform your therapist if there is paid parking at the desired treatment location, if you do not prepay for parking in advance, the payment will be due at the time of service.

  • It is important to have a space that will provide adequate room for the massage therapist to set up the massage table and comfortable maneuver around the area for your treatment (approximately 3 feet of space around each side of the massage table.)

  • Due to COVID-19, All outcall clients will be responsible for providing all table linens, pillows, etc and must be the only person present during the massage session for the safety of therapist and client.

Sanitation Procedures

Prior to COVID-19, Lavender & Lotus Therapeutic Massage adhered to strict sanitation policies set by the heath department and massage therapy board of Idaho.  We have implemented enhanced sanitation practices to ensure the safety of client and therapist.  Our sanitation protocol is as follows:

 

 

  • Clean linens, blanket, and towels are provided for all clients and are laundered after each use.

 

  • All equipment, including the massage table, bolster, lotion bottles, hot stones, stool, and any other tools used to provide services are sanitized with an OSHA and EPA approved disinfectant.  We currently use a product called Rejuvenate which can kill viruses and bacteria within 1 minute of contact time.

 

  • We use a vinyl moisture barrier between all pillows, face cradle, table warmer, and table pads that can be disinfected between each client.

 

  • Therapist wipes down all light switches, credit card processing machines, devices, lotion bottles, door handles, client belonging receptacle, and any other surfaces that client or therapist could come into contact with, with an EPA approved disinfectant.

 

  • Soiled linens are stored in a closed receptacle away from clean linens.

 

  • Trash is kept in a closed receptacle and removed each day.

 

  • Restroom is sanitized with an EPA disinfectant after each use.

 

  • All supplies are stored away from open air to limit any exposure.

 

  • Massage therapist thoroughly washes hands before and after each treatment.  

 

  • Clients will wash hands before treatment.

 

  • Both therapist and client will wear face coverings during treatment.

 

  • Gloves and hand sanitizer will be provided for client and therapist in the treatment room.
